Pricing and Insurance frequently asked questions.
All out of pocket payments, copays and coinsurances are due at the time of service.
It is recommended that clients contact their insurance providers for information regarding the cost of copays, coinsurances, deductibles and EAP information. Client should be sure to have any authorization codes/billing forms for EAP sessions and also be prepared to provide a copy of current insurance card and information.
The following questions are provided as a guide to help you with speaking with your insurance provider:
- What are my mental health/behavioral health benefits?
- Do I need a pre-authorization number for my visits?
- What is my deductible and how much has been met this year?
- What is the amount of my co-payment per therapy session?
- How many therapy sessions does my plan cover?
- How much does my insurance pay for an out-of-network provider?
Clients are responsible for sessions that are not covered by their providers due to termination or deductibles that need to be met.
There is a $75 fee for appointments that are not cancelled 24 hours prior to scheduled time or for any session not attended without a phone call or email.
